Overview

As a Health Coach you will be supporting A4e with the delivery of the Work Programme contract where you will support the Advisors in identifying and effectively supporting those customers with health issues. The role of the Health Coach involves completing short focussed customer interventions and effectively signposting customers to relevant A4e partners and community providers. As a Health Coach you will be expected to support A4e with the promotion of the company and contract, particularly within the Health and Social Care sector.

What you will be doing

*Supporting key Advisors in the assessment of customers needs in relation to their specific health needs / barriers

*Encouraging and facilitating a culture of independence and personal ownership of the goal plan for each customer on the programme.

*Facilitating the use of individual interventions applicable to clients on the work programme through advice and guidance

*Providing advice and guidance to key advisors on the complex health needs of customers with health and social care needs and establish optimum route way options for customers from in house to community support.

*Supporting Advisors in their learning and development

*Maintaining ongoing support for customers and advisors, and helping to support customers sustain employment.

*Identifying the need for training for the customer base

What you will need

*A health qualification at diploma / degree level and a live registration to a professional body IE NMC

*Specialist health interventions experience

*Ability to use specialist clinicalknowledge, skills and experience in the assessment of customers' needs in relation to their specific health condition

*A person-centred approach in engaging with customers through a collaborative process of goal planning and as part of the wider Work Programme team

*To identify a range of standardised assessments and inventories suitable for the mix of referrals to the team i.e. allowing for ethnic diversity, language and literacy levels

*To identify a network of community-based resources and activities accessible to the needs of the customer group

*Ability to build effective relationships with any companies partnered with A4e, community providers and the wider services available to our customer base.

*Ability to build a network of suitable contacts within the community to ensure a robust connection for referral route ways, which will appropriately address health based need

To be considered for this role you must be able to demonstrate A4e's 'DNA' - Trusted, Passionate, Driven, Brave, Friendly and Caring and be flexible to work 37.5 hours a week between the hours of 8am until 8pm, Monday to Friday and maybe some weekends when necessary
