We are looking for a Health & Well Being Coach with a background in supporting individuals to achieve their health and well being goals!
Wellnesscoaching' can be defined as the application of life-coachingprinciples tohealthandwellbeing. Our Health and Wellbeing coachwill use skilful communication to support individuals to make life changes through working towards their personal goals.
This role would be suitable for an individual that meets the below criteria:
Do you have a background in empowering people to overcome barriers and achieve lifestyle improvement goals
Do you have an understanding of the psycho-social factors that may limit an individual’s well being, including mental health awareness?
Are you passionate about helping people overcome barriers to achieve their goals?
Do you want to work in a multi-disciplinary team who share the same vision?
If so, then this is likely to be the role for you!
This role has an evidence based emphasis on improving individuals general well being and life enjoyment through:
Shared Decision Making • Personalised Care and Support Planning • Enabling choice, including legal rights to choice • community-based support • Supported self-management • Personal health budgets and integrated personal budgets.
We’ll need your qualifications and experience of supporting people who may have complex health needs to enable then to live a happy, healthy fulfilled life.
This role is at the forefront of supporting our Island’s NHS Primary Care services working with multi-disciplinary teams within General Practice to include: Cowes Medical Centre, Newport Health Centre and Wight Primary Partnership (Brookside Health Centre, West Wight). There will be an element of flexible working and you will be required to travel between various Island sites.
A full current driving licence and use of a car is essential.
You will work closely with our wider healthcare partners to include our local CCG, secondary and community care providers. It is essential you have a friendly, pro-active ‘can do attitude’ and willing to work as part of a team.
