Full job description

As the Active Health Manager, you will be responsible for leading our new to be launched innovative programme of activities that will look to link our service with other services across the city to help support the wider health and wellbeing system. Reporting into the Head of Active Wellbeing and Sport your role will involve:

- Create a network of partners to source new unique users across Plymouth through a referral pathway using physical, social, and cultural activities (Active Health).

- To develop referral processes, both from health agencies and wider social prescribers, and self-referral processes.

- Cultivate a network of partners and collaborators to increase capacity for physical activity for both ‘in centre’ delivery and out-reach off-site activities.

- To create and embed a clear sustainable in-centre offer for the referrals to become physically active with a focus on those who will benefit most from an active lifestyle.

- To be the lead to go out into the community and promote the pathway across sectors such as local authority, charities, and health partners.

- To lead on delivery and maintain quality assurance of the Active Health pathway.

- Work at a strategic level within the local health and care system to understand and contribute to strategic health outcomes of the Integrated Care System and Local Care Partnership.

- To monitor, ensure data collection and report the impact of the Active Health pathway.

- To work with local and national partners ensuring the pathway represents best practice, is evidenced based and always developing.

Requirements

- 3 years’ experience working and managing people in the health/Physical Activity/Sport sector.

- Able to work at a strategic level within the local health and care system to understand and contribute to strategic health outcomes of the Integrated Care System and Local Care Partnership.

- Understand the concept of collaboratively delivered inclusive, person-centred care based on best principles of equality and diversity.

- Understand the importance of data collection, evaluation and client feedback in sustaining and developing lifestyle intervention services.

- An understanding of national and local priorities for health interventions and policy

- Hold a degree or relevant health or fitness qualification with experience in the field of health improvement (e.g. community health, exercise referral or fitness industry)

- Proven experience of developing meaningful working relationships at all levels with internal and external stakeholders, clients, and customers

- Significant and appropriate experience in developing Community and Wellbeing initiatives in leisure and other public settings.

- Proven experience of successfully implementing place-based community programmes with exceptional results.

- Awareness of all relevant legislative, regulatory and health and safety requirements in relation to the role of a Health Navigator, or crossover points between this role and other roles within the Sport and Physical Activity industry.
