We have an exciting opportunity for someone to join the award winning Cardiac Rehabilitation Team based at Wycombe Hospital. The Care4today Cardiac Health programme is a well-established service and the post holder will be responsible for co-ordinating the exercise component of the phase III cardiac rehabilitation service. They will facilitate and coordinate the organising and delivery of this health related physical activity programme to a high standard. This involves pre & post screening with functional capacity testing, of all cardiac rehabilitation patients at the beginning and end of a phase III programme. A key aspect of the role is to ensure patients are referred into the most appropriate physical activity programme, organising and developing group sessions and thus ensuring suitability for exercise to our programme and long term management.

The candidate will also be responsible for reporting and evaluating the programme, providing information to manage and communicate performance for onward long term management of patients cardiac disease. They are expected to provide senior clinical leadership and supervision to the cardiac rehabilitation programme.
