Liverpool Community Health NHS Trust is at the heart of the delivery of healthcare within the communities of Liverpool and Sefton reaching a population of approximately 750,000 people.

We have a team of over 3000 staff – 80 per cent of whom are practising health professionals including Nurses, Community Matrons, Health Visitors, GPs, Dentists, Dieticians, Podiatrists, Physiotherapists, Occupational Therapists and Speech and Language Therapists. We also deliver specialist Dental Health Care, Therapies, Medicines Management and Nutrition and Dietetic Service.

We deliver services to people within their own homes, health centres, clinics, walk-in centres, and intermediate care centres. Our staff and services come into contact with hundreds of thousands of people in our local communities every year and therefore are often involved not just with patients, but also their families, carers and the communities in which they live. This means our staff can help to understand patients’ holistic health and social needs, including education, prevention, early detection and self-care.

Our services enable people to be cared for in their own community and remain as independent as possible, rather than having to go to hospital. Many of these services are delivered out-of-hours, with 24 hour, 7 day a week access.

As a Trust, we value our team and provide a range of great benefits to all our staff, including:

- Salary sacrifice car scheme

- Cycle to work scheme

- Childcare voucher scheme

- Staff physiotherapy service

- CPD, access to training

- Flexible working

- Staff support services

- NHS employee discounts

So if you care about your communities and are committed to delivering great care then Liverpool Community Health NHS Trust is the place for you.

An opportunity has arisen to be part of an innovative multi-award winning service.

The Liveability Service is based upon a holistic preventative approach to promoting health, well being, and increased physical activity levels in older people aged 50+, so that they stay active, healthy and independent in their own homes for as long as possible.

We are looking for a dynamic individual in possession of the relevant exercise qualifications required for this post. This is a unique and exciting role which will take a holistic view of the health and activity levels in older people, promoting their health, activity, independence, and overall quality of life.

The post holder will be required to deliver exercise sessions for older people within a gym setting and at bespoke venues across the city, working under the direction of the Liveability Fitness Instructor/Coordinator. Following training, the successful applicant will participate in health screening, visiting patients in their homes in order to identify need, reduce risk of falls, improve physical activity levels, and reduce inequalities in health.

Working alongside partner agencies and other professionals from health and social care, the post holder will contribute in determining the needs of older people and subsequent referral onto agencies / services in order to address those needs.

The Health Motivator role is pivotal in identifying the needs of older people by use of a holistic screening tool. Working alone and as part of a team who are committed to improving health and quality of life for older people, the post holder will be required to have experience of delivering exercise sessions, experience of working with older people, a caring and understanding attitude towards their needs, and have some knowledge of services currently available for older people in the wider community.
