The post holder will be an integral member of the cardiac rehabilitation team. They will assess, plan, implement, monitor and evaluate exercise programmes according to the needs of the referred individuals. They will be based in the community (at leisure centres/community centres or at the client’s home). They will help to improve participants heart conditions and in doing so promote optional functioning to reduce the likelihood of further heart events. They will encourage participants to become independent exercisers and promote the incorporation of exercise into normal activities of daily living. They will encourage participants to reap the benefits of an active lifestyle such as increased confidence, knowledge and functional capacity.

The post holder must be able to manage a case load. Health promotion and education is central to the role. Some lone working is involved. The post holder will be working with patients registered to General Practitioners in the Trust.
